That could happen in a chart cast for polar latitudes, for a date in December/January or June/July. In that case, the obvious solution is to use whole sign or equal house.
Many astrologers favor whole sign or equal house for high latitude charts as a matter of course, because of how wonky quadrant house systems get there.
