Yes, it does :)
Anything conjunct the Ascendant is OUT THERE, like the Ascendant is itself - the planets is just highlighted in a way.

It would give intense sharp pluto features? somewhat like having it conjunct ascendant. I mean its pluto anyways...

There are a couple of major aspects in this chart that affect appearance. But one thing is sure: this person's appearance changes. and also there is some unusual feature..maybe the eyes....but not negative, or not permanently negative....

Pluto only makes one negative aspect in this chart and that's to Venus.
I'm thinking, did this person have an accident which altered something about the appearance...or was there a genetic issue which affected the appearance?
If surgery is indicated, I see it as positive.
More specifics could give more info.
