Uranus is a co-significator as it is in the first house. I don't think this position
is very favorable, as Uranus is retrograde. I'm in a position of chaos.
I am not sure about his significator. 7th House? The 7th is ruled by Scorpio thus his significator would be Pluto (planet of death and rebirth...). Pluto is in the 10th house in opposition to Mars. Health breakdown? From what I know this is not a good aspect.
Moon is in the 2nd house which in terms of illness might mean that it could go either way.
I don't really have many sources on this subjects and most of the information I see online is on relational horary questions. Any good sites/book recommendations are welcome.

Hi Carole, I understand why you said that about Saturn in the 7th house. However, to correct this, just cast a sunrise chart for the "day" of the question, and then work from there. Then the horary question chart can be used as the secondary chart to triangulate.
Here are my thoughts ~
The "sunrise" chart of the day of the question, Sept. 29, 2007, shows Mars at cardinal point 90 ~ or, zero degrees tropical Cancer. Mars in the Ninth House, near the MC, is trine to Mercury at 1-SCOR, and applying as well in water trine to Pallas, and the Dragon's Head. Life and death issues, medical matters, questions, needs, wants, and desires surrounding the question of a man's health.
The Moon in the sunrise chart is in mid-TAU and square to Chiron... unanswered questions, worries, about a suffering person. The Moon is bi-quintile Neptune, which can be useful, but here, there is doubt, unsure about directions, how to proceed. The fact that transiting Moon is contra-parellel to Neptune complicates the matter further.
He is represented by Mars near Nadir (IC) in the horary chart ~ Mars will transit here between now (Oct. '07) to early May 2008. At this time, Mars is in trine to Mercury/SCORPIO, transiting the Eighth House. Mercury is quite active here, and a solution can be worked out during the month of October and November 2007. There is hope in finding a transplant.
Jupiter is moving towards a sextile with Neptune this month after seperating from a sextile to Chiron: doctors can find a liver but there are challenges (Jupiter square Uranus) that have to be navigated.
What will cause this to turn out for the positive, is persistance.
Mercury's transit, its station retrograde (Oct. 11) and its station direct (Nov. 2) will be tenuous. This means lots of communication, and planning for a transplant. His age, his own transits ~ especially secondary directed progressions ~ will reveal even more.
From the birth data you gave, what stands out is that transiting Saturn is conjoined to his natal South Lunar Node in Virgo. Transiting Saturn is opposed to his Mercury, which is conjoined to his North Lunar Node in Pisces (hard to communicate feelings).
He is having a nodal return, which arrives every 18/19 yrs., more or less, and with his natal Mars/Jupiter synod square to his lunar nodes, along with the Saturn's transit, and it's opposition to his natal Mercury the medical situation really needs to be corrected as soon as possible.
Venus, Jupiter and Saturn are most angular in this chart. This reflects the urgency, as Venus moves towards conjunction to Saturn, and the Dragon's Tail this October. I don't want to scare you, but it will be close if the doctors are unable to find a transplant soon. He most likely does not want to be scared, or to scare you, so, be at ease with him, and he will talk to you.
The water trines from Mercury (tropical Scorpio) and Mars (tropical Cancer) in October, and then again, in November 2007, really can help to ease things, while working to improve his health. It would help to have the medical condition improved this autumn, before the coming retrograde of Mars in mid-November, and the coming square to Mercury in mid-to-late December 2007.
