From my understanding of the subject, this indicates possible deficulty in conception and a likelihood of few (rather than many) children; however, numerous other factors in the chart, and also relating to the 5th house and Saturn itself, enter into the equation, and at best, even my above description is not much more than a generalization.
In Medieval Astrology you look at the planets in the 1st, 5th, 7th, 10th and 11th houses for children. You look for the fruitfulness of the Asc and the cusp of the 5th. Saturn in the 5th could mean that you have children later in life. It could also mean that you have a Saturnian child. You really have to look at your whole chart before you come to any conclusions.
Then it all depends on what 'house system' you use, Equal (my favourite) or placidus the default on astro.com

Reserved in their attitudes towards Art, Theatre, Pleasure. · Women – could be long time in labour
Saturn could be in 4th or 6th house

Unfortunately, there is no set formula or signature for marriage or childbirth, although others may disagree, apparently horary and vedic seem confident in this area. Your chart is a blueprint of your personality, facets and penchants and the whole chart is 'potential' for development. There is nothing guaranteed or fatalistic in charts, I do believe we still do have free will. I also believe we chose our parents and childhood circumstances best suited to learning those Lessons in the best way we can.
I think it's worth bearing in mind that the natal chart, like solar returns is all about 'potential' but as we all know we don't always fulfil that potential do we.
It's like someone saying you can predict death, but with modern medicine and sciences the way they are and the potential to 'die' three times in a life, doesn't mean to say you will does it
