You have a few indicators of a difficult life so far, but you do have positive attributes that you can take advantage of, as well.
With your sun in Aries square Mars in Cancer, probably you feel a lot of internal pressure. If you can't discharge it through some constructive means like vigorous sports of some kind, you are likely to get into arguments or even fights. With Pluto square Saturn, you may even have a bit of a cruel streak. So if you haven't been in trouble with the law so far, good for you!
Was your mother a particular problem for you? You would seem to have a lot of resentment towards her. The third house deals with siblings, so if you have any, I think they caused you a lot of hardship, as well.
Also, you may have had problems with drugs or alcohol, with your sun square Neptune and Neptune conjunct your moon. It is hard for people to maintain a strong sense of identity with these aspects, and you may have found that chemical highs took you out of the bad days to some extent. If you haven't gone down this road, I congratulate you, because you are likely to be extra-sensitive to drugs or alcohol.
So what to do? A blind chart reading is always hard, because each symbol in a horoscope has different meanings, and it doesn't say anything about the circumstances into which you were born, like rich or poor.
It seems to me that you are taking a very positive step now, in asking what is going on in your life.
As hard as it may sound, I think your key is some kind of discipline. With your sun (identity) sextiling Saturn, you can do it if you are determined. This might be something as simple as taking a karate class, getting yourself into an Outward Bound kind of program, or even joining the military. No, don't get blown up in Afghanistan! But you are a woman warrior, and you just need a constructive, socially acceptable way to express your inner rage--and your considerable courage.
Obviously, if you need to look after your health in some way first, you should.
Also, it looks to me like you are good at working with your hands, so learning a trade or taking up some kind of craft or art where you can make something should be beneficial.
