I cast the chart for moment I read your question:
9 feb 2012 19:00 Beograd Srbija ASC=14Vi34.
To have children ruler of 1,5,7,11 and Moon must be in fertile signs.
Ruler of 1 house Me is in neutral sign
Ruler of 5 house Sa is in neutral sign
Ruler of 7 house Ju is in neutral sign
Ruler of 11 house Moon is in barren sign
Ruler of Moon house Me is in neutral sign
As you see noone testimony is in fertile sign, and with one testimony in barren sign I think there is no chance to have children or they are really minimal.

Using Fensi88's chart, I thought I would approach your question using Lilly's rules for "If a man shall have children by his wife", the rules of which differ from the method used by Fensi88.
Lilly tells us to first look to the Ascendant, its ruler and the Moon. If the Lord of the Ascendant or the Moon are joined to the 5th by aspect or translation of light, then it either comes to be directly or after a space of time.
In this chart, Mercury, the ruler of the Ascendant is applying to the ruler of the 5th, retrograde Saturn, by trine. Mercury is in Aquarius, disposited by Saturn and Saturn is in his exaltation of Libra, creating an excellent reception between the two planets. This would serve to indicate that you will have children with this woman.
However, Saturn represents delays, obstacles and the circuitous route to a goal, all of which are already apparent in the events that have lead to this question. Moreover, Saturn is retrograde, which will intensify his ability to delay and withhold that which is desired. Saturn rewards tenacity, so my advice is to keep trying. I should also mention that Mercury, lord of the Ascendant, is weakened quite a bit by his combustion, and his presence in the 6th house of illnesses. The Part of Fortune is also there, which is another indication of difficulty. So, even though there is a perfection, there is quite a bit of struggle involved in bringing a baby into the world.
Lilly says next to consider Jupiter. If Jupiter is in the 1st, 3rd, 5th, 9th or 11th, free from affliction then the woman should be able to conceive without problem.
You have Jupiter in Taurus on the cusp of the 9th house, completely unaspected (I don't use out of sign aspects, in this case the opposition to Saturn, ruler of the 5th at the end of Libra), which gives a boost to the question in my opinion. Taurus is the exaltation of the Moon, and can often work well in questions regarding fertility.
Putting Lilly aside, I just want to make a few more observations. Virgo rises with the Moon right on the Ascendant. Mars is also occupying the 1st house. Leo and Virgo are the sterile signs of the zodiac. The Moon rules the 11th house, representing your hopes and wishes. The Moon naturally signifies motherhood. The Moon always co-rules the question and shows where our concerns really lie. This concern becomes especially apparent if we turn the chart. For the 11th house is also the 5th from the 7th house, your wife's fertility and children. You are concerned about her fertility. Pisces, a fertile sign, is on the cusp of the 7th, representing your wife. Cancer, another fertile sign, is on the cusp of the 11th. However, the Moon is in sterile Virgo with Malefic Mars, ruler of the 8th. Thus, the miscarriages and other problems mentioned. Also, if we look at the 2nd house, or the 8th from the 7th, your wife's reproductive organs, we find Libra on the cusp with a retrograde Saturn occupying the house. As previously mentioned, Saturn impedes and blocks. If you haven't already pursued thorough medical examination, I would suggest doing so. Saturn is cold and dry, a condition not conducive to bringing a child to term. Also, Venus, ruler of Libra, is in hot and dry Aries, her detriment, further bringing attention to dryness in that region. Perhaps there is an issue with amniotic fluid which creates a less than ideal environment for gestation.
All in all, it does not look like the path of fertility will be an easy one for you, but in my opinion it does not look completely hopeless either. Please keep us posted in regards to this situation.

There is an astrological method that can - by focusing on your most fertile five days each month - be helpful in predicting those five days each month when your wife is most likely to conceive
Begin by creating your wife's free natal vedic chart on astro.
You can do that on the Extended Chart Selection page by selecting the following options:
Whole Sign House system
Lahiri Geocentric Sidereal
Putting a check mark in the box that says "draw no aspect lines" (lower right of the page)
After that (on the same Extended Chart Selection page) select “special charts” then scroll down the pop up menu (that appears when you click on the small triangle)
You'll notice that the last choice is “Lunar Phases Fertility Calendar” - select that and then click to show the chart.
A twelve month calendar with your likely conception dates - based on your astrological information - is displayed.
Conception dates are indicated in red. Either a Venus or a Mars logo above each area accentuated in red indicates whether a child conceived at that time is likely either girl or boy
The idea is Venus = Girl
Mars = Boy :smile:

Thanks for bringing our attention to this excellent tool!
However, it would seem that conception is not the problem with this couple, as they have conceived at least three times. Bringing a baby to full term seems to be the problem.

That's ok agarth, the Lunar Phases Fertility Calendar can be useful!
Since conception is not the problem, then perhaps it can be helpful when utilised in tandem with some form of 'Electional' astrology - in order to determine how well the conception aspects relate to both of the parents natal charts. Hence a baby conceived at a time of good aspects to the parents charts may well be more likely brought to full term. Just an idea!

I like that idea very much! It would probably also be good to look at the wife's natal chart for indicators of fertility and the state of the reproductive organs as well, to see if any problems are noted there.
Cheers!

Good that you like the idea - it just occurred to me and seems to make so much sense - probably the idea has been thought of before, although I have not seen it recorded anywhere, so I have no source for it!
I agree agarth that indicators of fertility as promised in the natal chart - particularly now by progression as well as transit - are a good way forward to work with this problem astrologically. Combining dates of any possible medical interventions with positive astrological indicators as well

I am a student of horary and you seem to really know your stuff; I'm impressed! Question: You said that Leo and Virgo were the sterile signs of the zodiac. Also, you do not use out of sign aspects. I wanted to know if there is any reading you can recommend to further learn these ideologies? Please understand I come from a place of learning not criticism.
Thank you

Hi Serafin5,
Thanks for the lovely compliment!
Most horary books written by authors before 1700 provide the student of horary with the best foundation. Of all of these, "Christian Astrology" by William Lilly towers above the rest for its exhaustiveness. You can also look at John Gadbury "The Doctrine of Horary Questions" and William Ramesey's "Astrology Restored". Also, Lee Lehmann's "The Martial Art of Astrology" is excellent and highly recommended. Many find John Frawley's books to be insightful as well.
Cheers!

Thank you! I will check these out asap. Keep posting ok? I learn a lot from good horary astrologer's that explain as they go like yourself.
Gratefully,
