I have the worst health and luck in the world, but I would like to have surgery to fix my problems. My health problems can be seen in my natal chart and it is very accurate, but I can be a little vain and would like the change. Will I get surgery in the future?

(Following is not in accordance with standard horary practice and uses whole sign house format)
I'll delineate this using the Lot of Operations (ascendant+jupiter-sun) as primary indicator of the answer to this question:
-Lot of Operations (LOO) falls @ 7 Libra in the whole sign 10th house of advancement and elevations (and of course angular) = + testimony
-querent's significator Saturn (lord of the first whole sign house) is in orb of conjunction with the LOO = + testimony (note that querent's significator Saturn in this question is in its sign of exaltation, and angular-like the LOO-in the 10th house)
-querent's co-significator the Moon, flows toward the LOO = + testimony
-lord of the 6th house (querent's health/illness), Mercury, also flows toward the LOO which also = + testimony
In my opinion from this delineation the answer to the querent's question is a clear "yes"...

Sorry, I didn't know that a horary shouldn't be in equal houses. Do you think I should post one without equal? So, it suggests that I would be getting a lot of operations/surgeries. Do I need another chart to know if I would like my results? I wonder which surgeries because I would like at least 3.

95% of standard/horary practitioners erect horary charts in a quadrant house system, especially regiomontanus. I do not do so simply for one reason: because I have found horary in whole sign house to give (me) more reliably accurate delineative results. Actually horaries (according to standard horary) are not supposed to be in equal or whole sign house format-but I take exception to that because the first generation of horary pioneers always used whole sign houses (see Robert Hand for historical references)
The above chart (in my opinion) indicates you will have required surgery and this surgery will be successful-it does not address how many such surgeries will be needed, which question-again in my opinion-I don't think is really necessary at this point in time, but, later, and depending upon the outcome of the first surgery, such a question could become a valid one, in the future.

Can the horary tell you what kind of surgery or that would be pushing it?

It could but that would require a seperate specific question and, if one has an idea of what type of surgery it might be, it would be better to ask about that (more as a yes/no question) than just asking a general question: "what kind of surgery will I need" would only be valid if the person had no idea of what type of surgery they might require.
